Symptom:

The form wont save, also there may not be the calendar icon next to date fields

Cause:

Either the page has not loaded correctly, or the user has not got AMS in their compatibility view

Solution:

Try Ctrl + F5 (refresh), if this still does not load correctly send the user this document:
